Anyone heard of a code for CRRAFT or CRAFFT? :
 
CRAFFT is an acronym for an alcohol/substance abuse screening tool. If the screening included a brief intervention, code 99408 is appropriate (must meet time requirements).

99408 Alcohol and/or substance (other than tobacco) abuse structured screening (eg, AUDIT, DAST), and brief intervention (SBI) services; 15 to 30 minutes

Otherwise code 99420 for health risk assessment might be applicable. Payment policy will vary among payers.
