Scenario:

Pt is discharged from hospital and goes to a skilled nursing facility. Pt goes back to hospital for more than 3-4 days. Pt goes back to skilled nursing facility. What is the correct CPT E/M code for the physician to use when he/she evaluates the Pt at the SNF the second time?

CPT for SNF

your CPT code will be the appropriate level E/M for a follow up visit - 99307-99310. patient is not being re-admitted, they are being returned to the facility following a hospital stay.
