Wiki cranial nerve II/Ophthalmoscope exam

Hello, When an exam is performed to assess cranial nerves II, is the ophthalmoscopic exam considered part of the exam, or should it be documented separately? For example, if the statement is made: "the remainder of cranial nerves II through XII are intact? is that sufficient documentation to count the Ophthalmoscopic bullet in the exam?
