I have an op note where the doctor did a "crash primary mid-transverse cesarean section without proper sterile techinique" due to umbilical cord prolapse, non-reassuring fetal heart tones, unstable lie and polyhydraminos.

I have never coded a "crash" c section before - should I add modifier 22 to 59510 and send the op notes or should I just bill 59510 with no modifier or notes?

Thanks for any input :)

I would vote no on the 22 modifier. It may have been an "emergent" situation but it doesn't meet the criteria for modifier 22. This type of C section(not planned; due to fetal distress etc) is not that uncommon.
