I'm not sure if this is the correct thread, but I have a credentialing question. Our provider is an infectious disease doctor with his own practice. He also goes to different hospitals to provide care. When his wife filled out the 855B form, she added the facilities under additional practice locations. She put the facilities' NPI numbers and they sent it back and told her the doctor's NPI needs to be put on there. I do not do credentialing so I told her I would try to find out if this was the correct information. When we bill, we are going to put the facilities' NPI numbers on the claim as well as his NPI and name under the provider section. I hope this makes sense.
