When you are auditing an e/m visit and the provider has in the HPI Hyperactive thyroid, Anxiety, Depression would you categorize the Anxiety and Depression as 1 chronic condition or 2? So you would only give credit for 2 chronic conditions in the HPI or 3??

If the provider documentation the status of each of the conditions then I would count anxiety and depression as two separate conditions. For example:

- Anxiety; patient indicates anxiety has improved with the on going use of medication

- Depression; patient states depression is about the same with no improvement or worsening

Each chronic condition requires a status, so if they are listed separately with each condition and status noted then it meets the criteria.

They document Hyperactive thyroid helped with medication, Anxiety helped with medication, Depression taking medication as directed-Do you feel this is sufficient?
