When billing critical care is a statement needed by the Provider for rationale of critical care with the time spent per guidelines? I am new to this and a Provider is disputing the rationale is needed if he is documenting CC time spent on the patient.

As long as the documentation supports criteria for critical care then the CC time is all provider should need to document. See Medicare claims processing manual Chapter 12- 30.6.12 for critical care guidelines.
