Wiki CRNP coding at SNF

I am working with CRNP's who see patients in a SNF and they sometimes code several things at a time and I don't know the correct way to code it. For example; 99309, G0438 and G0506. How would I code this? Can these be coded together? Any help is appreciated. There are more examples but I don't have them available at this moment. I will put some more out there soon. Totally confused.
