So many changes over the years, is this coded per eye, per eyelash or per procedure? Example: if the procedure was done on the upper and lower of right eyelashes and the left upper eyelash. What modifiers should be used. Modifier 50, E modifiers, Lt & RT, XS. Getting a Medicare MUE that only one unit can be billed but this is the Blues. Can anyone provide clarification? this code has an MUE edit of 2-- I can tell you my blues on the east coast like the E modifiers-- so for bcbs I would do 67825 E3, E4 and then 67825 E1 and then make sure the attached dx are specific to the lids
