Does anyone know which reconstructions are REQUIRED in order to bill for a CTA versus CT Chest with contrast?

All CTA exams, chest or otherwise, require 3D post processing. If 2D (sagittal, coronal, multiplanar, etc.) reconstructions are done, code as a CT.
