I know you can bill physician services for both when the Dr. discharges from inpatient and transfers to Swingbed, but don't you have to have a separate note for each. Also what about discharging from Swingbed and admiting to inpatient acute care. Can you bill both the D/C from Swingbed 99315 and admit to acute 9922x. Also, my main question is do you absolutely have to have a separate note with an exam documented to bill the admit to acute. We do have a discharge summary with an exam from Swingbed.

We have conflicting answers among coders. Your expertise in the matter would be appreciated. Thanks.
