Would it be appropriate to add CKD unspecified (N18.9) to D63.1 if the CKD dx is not included on the order?

If someone has sent you this, then they've coded it for you already, and coded it incorrectly. I'd send it back to them to fix their mistake. There's no way to choose a code without documentation.
