Can coders count data(1 xray ordered) then use it again to count as an element of risk? (office op visits)

DATA order of 1 test xray- and again in the risk from additional testing/treatment because there's a risk to having xrays? This is without the provider stating this in the MDM-
Is this double dipping?

Data as a category I believe should be counted as data (xray or other imaging). I think considering an xray as a risk to the patient would be stretching it unless the patient had a comorbidity thay would create an actual possible risk, an example which isnt really xrays would be a pt with low renal function getting a CT scan with contrast. To note as well xray risk to a healthy patient is very minimal unless they are getting xrays all the time.
