If the doctor documents that he is requesting records from another physician, and also that he obtained information from a relative that was with the patient, how many points do you count this?
Also, does the additional information from a relative with the patient count as 1 point or 2 points?

Hi robin to your question "A DECISION TO OBTAIN OLD RECORDS OR ADDITIONAL HISTORY FROM SOMEONE OTHER THAN THE PATIENT IS WORTH 1 POINT.

thanks Brenda. That's what I had, but then when the doctor questions you, I needed to just reaffirm my knowledge.
