Wiki DC from Acute/ Admit to SNF same day

When a patient was discharged from Acute care and admitted to SNF (same hospital), I always billed both 99238/9 with a 25 modifier and then the SNF admit.

I was looking for supporting documentation and I found it just after submitting this!
