Wondered if anyone could help me with the following scenario:

I have a patient that has avulsion injuries on forearm. The provider states the following: "3x4 wound skin is torn, I debrided the skin off".

Is there a debridement code that qualifies to use or would this procedure be included in the E & M code? This is not necrotic or dead skin.

The provider is using 11042 and I feel like this is really not correct. Can anyone give me advice on this matter. Thanks in advance.
