Wiki debridement in global period

it is my understanding that debridement in the global period is only payable with a 78 mod if there is a return to OR not in office setiing. please advice
 
I am not getting paid even with Modifier -78. Am curious what other coders have to say also, I have used CPT 11042 and 97597, and both have denied.
