Wiki "decentralized" and "centralized process"?

Can anyone explain to me what is the diference between "decentralized" and
"centralized process"? and what the physicians will no longer do if their practice moves from the first to the second?

Are you asking due to the July 2007 self-test quiz questions? I looked for quite some time in the book and finally found it - it was in the interview with the person from Maine. When the office moved from a decentralized to a centralized process, the physicians would no longer be entering their charge information.
