Decision for end of life/hospice

I have no experience in this kind of coding, but a patient was seen in a nursing facility for multiple problems that were getting worse and the decision to call hopsice was made. The patient ended up dying 2 days later. How would I code this? Do I just code his conditions or is there a Dx for the decision for Hospice? Thanks in advance for any advice!

You don't say which provider or what services you are billing for and that could make a difference. You can take a look at V66.7 Encounter for palliative care, End-of-life care, Hospice care, Terminal care.
