Wiki Degree of Episiotomy

Our OB physicians are notorious for stating 2nd degree episiotomy, 3rd degree episiotomy, etc.

Are we to assume this was an episiotomy with a tear to 2nd degree, etc?
Should we query the physician to ask if this is an episiotomy with tear?

Query the physician. A tear and an episiotomy are not the same. Episiotomy, and it's repair (by the same physician) is included in the delivery. A tear is a complication of delivery. If the physician performs an episiotomy and the patient tears beyond the that, the documentation should clearly reflect that situation, with the resulting degree of laceration.
