Wiki Deleted 0375T - Coding HELP with 3 level Cervical Arthroplasty

Surgery in an ASC setting surgeon completed a Cervical Total Disc Replacement at levels C3-4, C4-5 and C5-6. Prior to 01/01/2020 would have used CPT 0375T but since the deletion of that code it leaves the question of what is the correct way to bill moving forward. The initial thought is 22856 (1st level), 22858 (2nd level) and 22899 (3rd level and beyond) but my surgeon states that only should be 22899 - replacement for 0375T.

Any one have definitive answer on which is the correct please and thank you...
