Hi everyone! I'm new here and was hoping you could help me out!

Our practice was coding 99090 for reading a smart card. That code has been deleted. We are trying to figure out a good replacement, if there is one. Code 99457 is being considered, but the card is not read remotely. It's read in the office. Also, there's a note that says to not count any time on a day when a physician is reporting an E/M code, but we are seeing the patient and will be reporting an E/M code. So, I guess my question is do we consider this as a replacement code or can we not do that since we are reporting an E/M code?

Has anyone else ran into this issue?

Thanks!

Services that were reported by 99090 are included in the E&M. They were included in E&M even when the code existed thus the code was RBRVS status B bundled.

Does anyone have a 2019 CPT Rationale?

To be able to confirm AMA's intent with deleting 99090, so that we may better define for our docs other appropriate coding/bundling and not just a "sorry, it's deleted" reply?

I don't have access to the CPT changes book but I was able to find the following based on the following links. Medicare did not pay separately for this because its a component of E&M just like wet read of x-ray, review of labs etc. Code was deleted because it was rarely used and didn't fits CMS need. Now this is only reportable under chronic care remote monitoring benefit with new and changed CPT codes
