If provider delivers a patient prior to 37 weeks (33 wk gestation) can we code preterm delivery In a sense of provider statement?Or does the provider have to state “ preterm delivery” in the op report. assuming Ptl is not listed as reason for admission. O60 preterm labor in icd 10 states : includes onset (spontaneous) of labor before 37 weeks of gestation. So does that mean we can “ interpret” a gestation prior to 37 weeks as preterm…

Because the code has a specific World Health Organization definition you may assume preterm delivery prior to 37 weeks. Here is how WHO defines this:

Preterm is defined as babies born alive before 37 weeks of pregnancy are completed. There are sub-categories of preterm birth, based on gestational age:
  • extremely preterm (less than 28 weeks)
  • very preterm (28 to less than 32 weeks)
  • moderate to late preterm (32 to 37 weeks).
But we only have a single classification for preterm birth and that is the code would need to assign if you know the baby delivered prior to 37 weeks. You are not interpreting the note, you are following the definition as stated in ICD10.
