Scenario:

Patient delivered c-section but at the time of the incision a foul smellin odor was detected.
Diagnosis was chorioamnionitis/endometritis. Patient in the hospital for 9 days to treat this infection. How do we bill for the E/M services for those days as normally global CPT 59510. What modifier would be used? ICD-9 Dx code 658.41 and any other dx?? Also patient developed an abscess by the incision. Central part of the Wound was then reopened for drainage. Can we use just I&D code 10060 as there is no CPT code in the OB section and would this be considered a post-operative injection as already had infection at the time of delivery. Any help would be appreaciated. Thanks

Delivery extended stay

I would code 65843 if Chorio happened before she gave birth. Code 65841 if Chorio happened during labor.. No separate E/M code.

Endometritis code 67012 and give E/M if happened after delivery.

Infection of C-section..code 67432 and 6822 if cellulitis is mentioned if not code 67432

This would get a separate E/M code
