In a pediatric office, we currently have a policy that requires patients to update their demographics yearly.

In our demographic packet we include a document for the parent to sign that gives other individuals the consent to treat their child.

In this yearly update, is it required to have the parent fill out another consent to treat document? Or will the document automatically apply year after year until it's revoked in writing?
 
This would depend on the wording on the document and whether or not there is an end date. You may want to contact counsel regarding your form and their recommendation on how often it should be re-verified and to confirm that wording is appropriate and that your provider is protected.
