Hello,

I first billed out 92928 RC, 93458 26 XU,92978 XU LM, and 92978 LD. Got the denial for 92978 and 92978. I changed one of 92978 to 92979 and removed the XU then added modifier 26. Now, they have denied 93458, 92978, and 92979. Any suggestions on how to get this paid ?
 
I have never used the XU modifier. I would code these 92928 RC 93458 26 59 92978 26 LM 92979 26 LD
