Hello I billed 92937 RC,93459 26 XU, and 75710 26 XU and received denial on 93459 and 75710 as inclusive. Any suggestions on how to get this paid ?
 
Okay, the physician did the Left Heart Cath with bypass grafts angiography and the measurement to bill the 93459 and then the did selective angiography of left subclavian artery with LIMA catheter. The proceed with coronary intervention to saphenous vein graft in the RC.
 
The left subclavian could have been to see if the LIMA was good enough for a bypass graft. Was there any problems with the left arm vascular system?
