Wiki Denial due to 0250 for inpatient stay.

We received a denial from UHC stating that revenue code 0250 is only for outpatient services at a hospital, not for inpatient stay. We have never had a claim deny for this before. The medication was Ceftriaxone 1g with Revenue code 0250. They denied stating that revenue code is only for outpatient care at a facility. Anyone ever experience this before or can point at a policy we can respond with? TIA

Revenue Code 250 should be billable on an inpatient claim. You'd bill it with all the charges rolled up together on one line, no HCPC.

By any chance, was there a HCPC included on the Revenue Code 250 line?
