Wiki Denial for 37250? Help??

Most of my 37250 are being denied by all insurance companies. I bill them with stents in the peripheral vascular system. They do pay for the 37251 when billed and the 75945 and 75946 but 37250 is denied for qualifying procedure. I have no idea what the qualifying procedure would be, the stent? since it includes cath placement. Is anyone else having trouble getting these paid?
