We've gotten some denials from BCBS stating "procedure inconsistent with modifier used or modifier is missing". We're billing a flu shot (90656) and the administration (90471), some of the denials are only a flu shot with no office visit, a couple others are with an office visit which we have put the 25 modifier on. The only modifier I know of would be SL for a state supplied vaccine but the flu shot we're billing for is not state supplied. Does anyone else know of another modifier? Thanks for any help!
