Our office does venous ablation as well as phlebectomy and sclerotherapy. Humana has rejected every modifier that we have used. I have tried XS, XU, 59 and even no modifier. When we bill with a modifier they say incorrect modifier and when we don't use a modifier they say modifier required. Any suggestions? Humana is no help
 
Which codes did you use? Which procedures were done for what diagnosis? There are a couple of different codes depending on approach, diagnosis, number of incisions, etc. By randomly checking some of the codes I do not see any CCI edits so I do not see where you would need any modifier.

Please provide the codes you billed and possibly the note so that we can provide better assistance for your problem! :)
