Billed 99214-25, 87428 QW-59; 87880-QW; 36416-59; 85025-QW; 87651-QW. All were paid except 87651 - denied due to incidental to 87428. We've attached the QW, so I'm not sure why it's been denied. Any thoughts? Thank you.
 
this does not need a -25 modifier or a -59 modifier.
but 87880 and 87651 are both strep tests, so they (most likely) should not be billed together.
