Wiki denial on Canalith repositioning procedure with E&M on the same day

How is everyone else billing CPT 95992 and getting paid. Can this procedure be billed with an E&M or is it included in E&M?

95992 has a status code of B in the medicare fee schedule, meaning that it is considered Bundled in an EM when performed on the same date of service.
