Hi,

I have a scenario 92928 LD, 93458 26 59,93571 LD. Meridian Medicare Advantage is denying for the 93458 26 59 and 93571 LD. I am not sure why they are denying it. Any suggestions? Do you need another modifier? Or need to write an appeal letter ?

they didn't give you any reason for denying it?...like it was bundled or something?...if not I would not try to figure it out I would call the payer to get specifics...
