Wiki Denials with Inpt admits and D/C with prolonged service

I work for a multi specialty practice (pulmonologists/intensivist's/hospitalists) Currently we are experiencing denials when billing the prolonged service with inpt admits and discharges. this is from multiple carriers across the board. Has anyone experienced this? Thank you.
