I billed 32551 and 99255 on the same day and it denied. Should i have billed 99255/25 with 32551 to pay. I am new to hospital in pt billing. Thanks
 
Without the actual note it is hard to advise on what you should have billed. However with an E&M and a procedure on the same encounter, the E&M does need the 25 modifier to be payable. However your documentation does need to meet the requirements for the 25 modifier.
