Wiki Denied HCPCS codes J2405, J7040, J3490, J3420 need help

Good morning!

So I got this denial note: "J2405, J7040, J3490 & J3420 denied: missing NDC. Need to submit corrected claim."

I searched what NDC means. So for this denial, do I need to contact the provider to scan the boxes of each drug administered to the patient? Or ask them to document the NDC when documenting patient charts?
