I have a provider that is injecting J1020 (20mg) bilaterally. 20mg on right side and 20mg on left side

Do I bill one of the following ways
J1030 which is 40mg
J1020-50
J1020-LT & J1020-RT
J1020 x 2

Any website that you can direct me to would be helpful.

You should code only the J1030 (40 mg) with 1 unit for the total 40 mgs injected. Drugs are coded based on the total amount given during that visit. That your provider decided to split the 40mgs into 2 injections (patient comfort?) doesnt mean you split the coding
