I have numerous depo provera claims being denied by BCBS for invalid NDC#. I am attaching the NDC# directly off the medication which is 59762-4538-2 or 59762-4537-1. BCBS is the only carrier that is denying the depo claims for this reason. Is anyone else experiencing this? Does someone else have a different NDC# for depo provera?

NDC number format

I think your issue is the NDC# is 10 digits and the official HIPAA format is 11 digits as 5-4-2. You would enter the code without any dashes. If any section has less than the required number of digits, you add a 0 at the beginning of that section. In your case, the format is 5-4-1 so you would add a 0 at the beginning of the last section.
59762-4538-2 becomes 59762453802
59762-4537-1 becomes 59762453701
