My doctors regularly give .5ml of 80mg/ml DepoMedrol how should I bill this? We have a discussion in our department. Some say J1080 because of the round up and NDC and others say J1040 because of the billing amount?
 
J1080 is a deleted code. However these are several codes for depo-Medrol:

J1020 20mg
J1030 40mg
J1040 80mg

If you are administering .5ml of 80mg/ml bottle you would bill the J1030 40mg unless the remaining amount in the vial has to be discarded. If so, then you would bill the J1040 mg with the appropriate modifier for discard (JW)

Thanks! I appreciate your reply. And I actually mistyped J1080. Our group discussion was about J1030 and J1040 as your answer indicates you surmised. I was of the mind to bill J1030 40mg and others J1040 80mg because of the "round up" The others thought the NDCs would not match the CMS NDC and the charge would be denied.

I need help please! I am not sure if this the correct way to code.

My doctor use 3ml of 40mg depo-medrol.

I code J1040 for 80mg and J1030 for additional 40mg.
