If a placenta is received for pathology review, labeled unspecified type of delivery at 35 weeks and the diagnosis states "focal areas of chorioamnionitis and increased meconium staining", which would be the best ICD9 code to use:

656.81 Other specified fetal and placental problems (meconium staining)
644.21 Early onset of delivery (35 weeks)
658.41 Infection of amniotic cavity (Chorioamnionitis)

I always have a hard time determining which is the main diagnosis anytime I have to code anything other than normal delivery.
