I am having difficulty with a current patient who had a mastectomy in 2013. Her left side has developed a skin flap that gets irritated and ulcerated at times. The doctor wants to do an excision.

I have been researching other conversations and one thread dated 12-18-2012 has used benign lesion excision code 1140X and intermediate repair 1203X based on size. Our patient just had a mastectomy and no reconstruction, are these still the codes that we should use? We have tried 15839 on other patients with denials. Also what are the appropriate ICD-10 codes for the skin flap?

Any help or suggestions will be appreciated! Thank you very much in advance!
