Our pediatric practice uses the PEDS response and score forms for developmental screens done at physical exams. I am looking for documentation that states that the PEDS score form (front & back) must be scanned into the patients chart along with the PEDS response form that the parent completes. Currently my providers state they only need to note in the chart the Path that was selected, not proof (the form scanned in) of how they arrived at that decision. Anyone that can give me some guidance would be a great help.
