Our physician's would like to use 250.92 or 250.93 rather than 250.40, 250.60, 250.50, 250.70, 250.80 and then also use the manifestation codes such as CKD, polyneuropathy, glaucoma is this appropriate? This is how our physicians think and would prefer to use these codes. If not could you suggest documentation that would prove why they cannot?
 
It is not appropriate to use unspecified when the documentation is specific. The documentation you seek is in the coding guidelines. It states you must always code to the highest level of specificity.
