How is this coded? Would it be E11.36 because the PCO is considered secondary cataract

No. H26.49 is the base code you use with additional digits for the eye(s) involved. In most cases, unless the diabetes is uncontrolled for a long period of time, the posterior capsule opacity isn't related to the diabetes.

The code you mention above is used when a patient's diabetes is uncontrolled and they develop an initial cataract as a result of that and doesn't have anything to do with the posterior capsule opacity.
