Can someone please direct me on the proper way to code Diabetes and manifestations. I have been audited recently and was told that the manifestation does not have to be documented to code a diabetes code with manifestations. For example: The physician documented Diabetes (250.60)w/polyneupropathy(356.9). There was no documentation of the manifestation so I coded the 250.0:confused:0. Am I wrong here??? Also would anyone please direct me to some supporting documentation.

diabetes with polyneuropathy does indicate a causal relationship. Sheck the coding guidelines for the definition of the word with when documented in this fashion. Therefore your physician codes are correct as is the order.
