If you have a DX of LE ulcers,Diabetis,CHF would you code the Diabetis as 250.00 or 250.80?
 
Based on a list of dx you cannot make ANY assumptions. There is no way to know if the diabetes is the casual condition or if it it is just a complicating factor. You must look at the documentation from the physician to see if he links the conditions, like saying the ulcer is due to the diabetes or this is a diabetic ulcer.. if it is documented as such then you will code the diabetes first as 250.8x if it is ulcers and he also mentions that the patient is diabetic with no causal reference then you code the ulcer first with the diabetes as the 250.0x
