about Vitamin D

We use in our lab billing the CCL edits information that is provided through the CMS website. If you do a search with the lab cpt code it should give you all the info need in regards to the dx codes that can be used for this procedure. Hope this helps....

If it is a screening then the patient is asymptomatic and you need a V code for screening. Medicare will either consider a covered screening or it will be patient responsibility.
If the test is being performed because the patient has symptoms, then you use the code for those documented symptoms. Again Medicare will either cover the test for medical necessity or it will be patient responsibility.
You cannot use a code not documented for by the provider for the specific encounter in question.
