Wiki Diagnoses for ASCs

So when billing for an ASC all the coder has is the OP note. The diagnostic statement says torn medial menicus. The coder picks up the injury code. However, in the office visit the surgeon specifically states that there was NO injury involved. My question is does it make a difference what diagnosis the ASC uses. I think that we need to use the non-injury diagnosis but I'm being told it doesn't matter and that the injury diagnosis can be used.

Thanks
