For Primary Care Physician office with in office x ray...f patient comes in with a cough and doctor performs chest x ray then determines patient has pneumonia do you code cough for the chest x ray since that was to ordering diagnosis and then assign pneumonia as the definitive diagnosis on the E&M. I know if the patient was referred out then the reading physician would code pneumonia but since in office would we use the the diagnosis that prompted the study and led to the final diagnosis.
